New Informed Consent Template & Changes to Stamping
A new consent form template has been published and is available on the Forms & Templates page.
Note that with this new template informed consent forms will no longer be stamped with an expiration date. The documents will still have an IRB approved date which will be located in the header of the document.
Current studies that use an older template will be modified during either an upcoming Amendment or Continuing Review.
Please ensure you are using the correct IRB Approved versions of all documents throughout your study.
